How It Works
Four Steps to an Open Account
- 1Download the Credit ApplicationFill it out completely — an officer of the company must sign it.
- 2Attach Your Exemption CertificateIf you’re sales tax exempt, include the correct completed form (see below).
- 3Email It to SalesSend the completed application to sales@sisupply.com.
- 4Account Review & SetupWe’ll follow up directly once your account is ready to go.

Credit Application
Two pages — company info, A/P contact, officers/principals, and bank/trade references. An officer signature is required.
Completed applications go to sales@sisupply.com.
Step 2 (If Applicable)
Sales Tax Exemption Forms
If your purchases qualify for a sales tax exemption, include the form that matches your situation. Not sure which one applies? Ask your rep — we’ll point you to the right one.
For Missouri-based resale, manufacturing, and wholesale exemptions — covers ingredient/component parts, manufacturing machinery, and several other exemption categories.
